New Patient Registration

To register with the Practice, call in at the surgery and at Reception for a Registration Form. Alternatively, the forms can be found by clicking on the 'Registering Online' tab above.

Once you have filled a form in and handed it back to Reception, you may asked to book in for a New Patient Medical appointment. This is a brief appointment with a Healthcare Assistant regarding your medical history, current height, weight, smoking status etc. If offered please try make every effort to attend this appointment. If offered it will only be to those new patients over the age of 16. 

Registration can take up to 3 weeks to process.

Note: Please ensure that all sections are filled out on the form. details such as NHS number or previous GP details can be essential to help us find you on the system. If you are new to the Country please make sure you add the date that you entered the UK. If details are missing this could delay the registration process.

Any new patients under the age of 16 will have to be seen by a member of staff before being registered, and can only be registered if a parent/guardian is a current patient at the practice.   

Note: We may ask you to provide 2 forms of identification when registering; one form of photo ID and one form of proof of address. This will help the practice staff ensure all your details are recorded correctly and reduce any future issues with your registration. Please note if you don't have these documents this will not stop us processing your registration.
