New Patient Registration
To register with the Practice, call in at the surgery and at Reception for a Registration Form. Alternatively, the forms can be found by clicking on the 'Registering Online' tab above.
Once you have filled a form in and handed it back to Reception, you will be asked to book in for a New Patient Medical appointment. This is a brief appointment with a Healthcare Assistant regarding your medical history, current height, weight, smoking status etc. Attending this appointment is compulsory for new patients over the age of 16.
Any new patients under the age of 16 will have to be seen by a member of staff before being registered, and can only be registered if a parent/guardian is a current patient at the practice.
Note: New patients will need to provide 2 forms of identification when registering; one form of photo ID and one form of proof of address. For European citizens registering, you will need to provide either your passport or National Identity Card.
